Acceptance Criteria
ALFA primarily serves residents of Clinton, Eaton, and Ingham counties. Applicants could be denied, and participants may be discharged from the program, for the following reasons: (1) persistent and uncontrolled incontinence; (2) persistent disruptive or violent behavior, including chronic "run-aways;" (3) need of physical care beyond the capabilities of the program. In fairness to all, priority is given to those living alone or with a family member caregiver. Those in adult foster care or nursing homes are given a lower priority since daily care is already provided. No one is excluded from the program on the basis of race, sex, color, religion, or national origin.

 

Procedure for Enrollment (Prospective participant may withdraw application at any point.)
Completion of ALFA application and completion of Tuberculosis testing Arrangement for visit(s) to ALFA with the accompaniment of caregiver. Arrangement for intake interview with caregiver and participant (often at 9 a.m.of the first program day) Attend first day without caregiver present

 

Tuberculosis Information
All incoming participants will be asked to present proof that they have been deemed negative for Tuberculosis (TB) before entering the ALFA program. This can be done in several ways:

1. Have a TB skin test and bring in a confirmation that an RN has read the test and found the results to be negative.
2. Chest X-ray found to be negative for TB, confirmed by physician's office.
3. Confirmation from a physician's office that either of the above has been determined within the last three years.

It has become accepted practice in the last several years to require such testing of anyone entering a group situation (schools, treatment programs, etc.) All ALFA staff, participants and visiting students are required to have this testing done every three years. Initial visits to the ALFA program may be done without the proof of TB testing.

 

Cost
Basic Program fee: $ 9 /hr.
For example: 5 hrs. = $ 45 program fee + $2 donation for lunch = $47
8 hrs. = $ 72 + $2 donation for lunch = $74 

Transportation cost is included in fee in most cases
